If your child does not yet have an autism diagnosis, As You Are offers a convenient, fully online evaluation process—no long waitlists or travel required. Families can schedule a virtual appointment with a licensed physician who specializes in developmental evaluations. After the assessment, you’ll receive a diagnostic report that can be shared directly with Beautiful Minds to begin ABA therapy services right away.
Physician Referral Form
Some insurance plans require a doctor’s referral before beginning ABA therapy. If your child’s provider does not already have a referral form, you can print and take ours to your doctor’s office or email it directly to your provider to complete and return. This form helps us gather the information needed to verify eligibility and begin the intake process quickly.
